特应性皮炎(AD)是慢性、复发性、炎症性皮肤病,常伴明显瘙痒,严重影响患者生活质量。轻度患者可外用润肤剂、糖皮质激素(简称激素)或钙调磷酸酶抑制剂治疗[1],中、重度患者常需联合免疫抑制剂进行治疗[2]。但传统的治疗药物往往存在许多局限性。随着近年来对AD发病机制研究的深入,用于治疗AD的众多新型靶向治疗药物被研究者发掘,这些药物对其目标分子具有较高的特异性,从而减少了免疫抑制剂非靶点效应所引起的不良副作用,给中重度AD患者的有效治疗带来了福音。
传统治疗药物的局限性
外用糖皮质激素治疗AD具有较好的疗效[3,4],但长期使用可能会导致皮肤萎缩、毛细血管扩张、皮肤脆性增加等不良反应[5,6]。钙调磷酸酶抑制剂通常作为外用糖皮质激素的替代药物[7],然而长期使用可能与皮肤肿瘤(特别是皮肤淋巴瘤和皮肤鳞状细胞癌)的发生存在关联[8-11]。中、重度特应性皮炎患者,可联合给予系统免疫抑制剂的治疗,但此类药物的不良反应较多且较为严重,例如肝毒性、肾毒性、骨髓抑制、致畸和抑制生长发育、致癌、消化系统反应等,需格外注意药物的潜在风险[12]。
中重度AD患者控制的困境
美国的一项真实世界的横断面研究对中重度AD患者的疾病控制情况进行了调查,研究纳入1064名中重度AD患者。其中625名患者(占全部患者的58.7%)被医生认定为AD整体控制不佳,包括急性发作、病情恶化或不稳定与治疗效果不满意。接受免疫抑制剂的患者中控制不足的占比为53.4%,接受全身糖皮质激素治疗的患者中控制不足的占比则高达83.4%.[13]

医生与患者报告的结果均显示,相较于病情得到控制的患者, AD控制不足的患者在心理健康、瘙痒和睡眠障碍对日常生活的干扰,及对工作效率方面的影响方面均表现出显著的疾病负担。回归分析显示,头/颈部、下肢部位的症状、瘙痒的困扰,及睡眠障碍等因素与患者的疾病控制不足显著相关(如图2)。

中重度AD患者治疗新的希望
中重度AD患者的治疗之路,仍然是困难重重,疾病难以得到有效稳定的控制。随着对疾病机制认识的发展,一些药物特别是新型生物制剂的上市,有望为中重度AD患者带来新的希望。如研究发现磷酸二酯酶-4(PDE4)是参与促炎细胞因子产生的细胞内酶,参与AD的发生和发展[14]。PDE4抑制剂OPA-15406治疗AD患者可显著改善湿疹皮损面积及严重度指数(EASI)和视觉模拟量表瘙痒评分[15]。

磷酸二酯酶-4抑制剂
磷酸二酯酶-4(PDE4)是参与促炎细胞因子产生的细胞内酶,参与AD的发生和发展[14]。2016年美国一项随机、双盲、对照研究评估了不同浓度PDE4抑制剂OPA-15406对AD患者的疗效,结果提示OPA-15406治疗8周后,湿疹皮损面积及严重度指数(EASI)和视觉模拟量表瘙痒评分方面均有显著临床改善,其中第4周时OPA-15406组平均EASI的改善最显著[15]。

度普利尤单抗
度普利尤单抗是一类全人源化的IgG4亚类单克隆抗体。2017年3月,美国食品和药物管理局(FDA)批准局部注射度普利尤单抗用于治疗成人中度至重度特应性皮炎[19],并于2022年得到中国国家药品监督管理局获批,用于治疗外用药控制不佳或不建议使用外用药的6岁及以上儿童和成人中重度AD的适应症。本品可与或不与外用皮质类固醇联合使用。
总结
虽然AD的病因和发病机制仍未完全阐明,传统的AD治疗药物存在许多诸多不良反应并且疗效不佳,但是近年来,随着对AD研究的深入,许多的新型药物研发的问世,特别是度普利尤单抗等靶向生物制剂,已经被证明治疗效果显著,不论单药治疗,还是联合外用糖皮质激素,均能明显改善患者的临床症状和体征,显著改善患者生活质量,为中重度AD患者提供了更好的选择。未来AD的治疗一定会有光明的前景。
